    • A plan records what was discussed and what happens next — nothing more
    • It is written to be shared with family or another clinician
    • If a line does not make sense, that is worth a message rather than a guess
    • Plans are revised at follow-up rather than replaced silently

    Making the most of follow-up visits

    Follow-ups are short by design. A little preparation makes the difference between a check-in and a useful conversation.

    • Re-read the previous summary the evening before
    • Note anything that has changed since, even briefly
    • Bring the same list you brought last time, updated
    • Ask for the next follow-up to be booked before you leave
