    Preparing for a visit

A short routine for the day before an appointment, so the time in the room is spent on the conversation rather than on recall.

    • Write down what you would most like to have covered, in order
    • Note when you first noticed whatever prompted the appointment
    • Bring the name of anything you take regularly, exactly as written on the packet
    • Decide whether you would like someone with you, and say so when booking

Questions to bring to an appointment

Four general questions that tend to make any appointment more useful, whatever it is about.

    • What are we trying to find out today?
    • What are the options as you see them?
    • What would make you want to see me sooner?
    • Who do I contact between now and the next appointment?

    Clinic visit checklist

The short version — what to have with you when you leave the house.

    • Your question list
    • Names of anything you take regularly
    • Any previous summary relevant to the visit
    • The location name — Clinic has four, and they are not interchangeable

    Bringing someone with you

How companions, chaperones and interpreters are arranged, and why it helps to say in advance.

    • A companion can be added to any appointment when booking
    • A chaperone is offered as standard and can always be requested
    • Interpreting is arranged in advance rather than on the day
    • Rooms are booked by size, so notice means everyone gets a seat

These notes are about navigating appointments and paperwork. They are not medical advice, they are not specific to anybody, and they are part of a fictional template.
